Application for a US Passport Name Change and Requirements

• Current Passport

• Please use this application if your passport was issued LESS than a year ago. OR Please use this application if your passport was issued MORE than a year ago. Upon completion, PRINT OUT the application. No handwritten applications will be accepted.

• Two-passport photos (2"x2") front view with a white background: Please Do Not staple your pictures onto the application.

• Copy of a valid driver's license or other official ID.

Original marriage certificate, original divorce certificate, or a court decree showing your name change information.

• Your own personal check or money order to the US Department of State. Please write you date of birth on the check or money order.

• Travel Itinerary. Ex: Internet confirmation letter from airline or a copy of your plane ticket showing date of travel.

Processing Times: Business days only M-F, no national holidays
