The Chicago Consulate has the states of Illinois, Indiana, Iowa, Michigan, Minnesota, Missouri, Nebraska, North Dakota, South Dakota and Wisconsin under its jurisdiction and is located at 401 North Michigan Avenue.

Brazil Tourist Visa Requirements and Application
When you start the Brazil visa application, click on the link that says "Visa Request" towards the bottom left of the website there. It's important to know that you will need to fill in all of the fields required or else they will reject your application as incomplete.
After completing all the required fields, electronically submit the application through their site. The next step is to print out the confirmation page. When you go to the consulate, take the confirmation page with the rest of the documents.
In addition to the Brazil tourist visa confirmation, you also need to turn in a confirmed travel itinerary showing the dates of travel. One passport-style photograph is required and it's best to pay for one instead of taking a photo yourself at home.
Make sure to check your passport. It needs to be in good condition with more than six months left before expiration by the time your trip is complete.
The only payment method the Brazil Consulate will allow is a money order from the post office. They will not accept any other type of payment even if it's a money order from another source.
Do you live too far from the embassy in your jurisdiction? If you are unable to apply for your in person, you can either have someone you know apply on your behalf or have a registered Brazil visa service take care of it for you.
