Lost Passport Replacement Requirements and Application

• Please click here to fill out the online DS-11 application. Upon completion, PRINT OUT the application. No handwritten applications will be accepted.

• Please click here to fill out the DS-64 application for a lost passport replacement. Upon completion, PRINT OUT the application. No handwritten applications will be accepted.

• Provide ONE of the following:

  * Certified Birth Certificate if born in US

  * Certificate of Citizenship if born outside the US

  * Certificate of Naturalization

• Two-passport photos (2"x2") front view with a white background: Please Do Not staple your pictures onto the application.

• Your own personal check or money order in the amount of $135.00 made out to: US Department of State. Please write the date of birth on the check or money order.

• Travel Itinerary. Ex: Internet confirmation letter from airline or a copy of your plane ticket showing date of travel. If you do not have an itinerary, we can help you out. Just give us a call. If you work at a Company that is requiring you to get a Passport in order to begin the processing of a Visa for business, please call us first for instruction on your business letter.

• Gather all of the above and take your documents to your local post office. Tell the representative you want to EXPEDITE using a passport courier service. The agent will check all documents, you will then sign your application and then he/she will place everything in a sealed envelope(except one copy of authorization letter and our service fee) and return it to you. Then please send us the sealed envelope and the loose third copy of authorization letter and service fee via FedEx.
