Passport name change question
by Walter
(Georgia)
I changed my name in June of 2008. My question has to do with my New York birth certificate. Will I need to have my birth certificate re-issued under my new name to obtain a passport? Time is of the essence.

Answer:Hello,
If you legally changed your name then you should have been issued documentation showing proof of legally changing your name. You will need to submit that original proof along with your original birth certificate in order to get a passport.
I'm not sure what the specific details are of your situation. If it's complicated and you are still worried about this then I encourage you to directly call the Passport Information Center. There number is 1-877-487-2778.
Based on your circumstances they will be able to give you specific instructions on what you need to submit.
