• Current Passport

• Two-passport photos (2"x2") front view with a white background: Please Do NOT staple your pictures onto the application.

• Letter requesting the need for a Second Passport. Call us if you need help on how to write the letter.

• Your own personal check or money order in the amount of $135.00 made out to: US Department of State. Please write you date of birth on the check or money order.

• Travel Itinerary. Ex: Internet confirmation letter from airline or a copy of your plane ticket showing date of travel. If you do not have an itinerary, we can help you out. Just give us a call.
